Add This Receipt Form to Your Phone

iPhone

1. Open this page in Safari.

2. Tap the Share button at the bottom of the screen.

3. Scroll down and tap Add to Home Screen.

4. Tap Add in the top-right corner.

5. A Receipt icon will now appear on your Home Screen like an app.

Android

1. Open this page in Google Chrome.

2. Tap the three dots in the top-right corner.

3. Tap Add to Home screen or Install app.

4. Follow the prompts to add it.

5. A Receipt icon will now appear on your Home Screen.

After it is added:
Tap the Receipt icon anytime you need to turn in a receipt. It will automatically open a new email with the receipt information ready for you to complete.
